To ensure thorough and timely service, please remember to bring the following to your first visit:
- All insurance cards
- Photo ID
- A copy of your relevant medical records (such as previous Urologist office notes, Recent Lab Results, Imaging reports)
- A complete list of medications (prescriptions and over-the-counter medications)
- Your referral form (if applicable).
Please note that if you are insured by an HMO care plan, it is your responsibility to obtain your referral from your primary care provider.
Please make sure to print out the New Patient Form and answer all the questions. We ask that you bring the completed form to your appointment.
If you are a new patient, we kindly ask that you arrive at least 10 minutes prior to your scheduled appointment time in order to process your information.
